Where can information concerning the origin and evolution of requirements be found?

The foreword of each Annex to the convention on International Civil Aviation provides historical background information concerning the development of the Annex. A table shows the origin of subsequent amendments together with a list of the principal subjects involved and the dates on which the Annex and the amendments were adopted by the ICAO Council, when they became effective and when they became applicable.

Where can a listing of all international airport codes be found?

A listing of four-letter indicators for geographical locations throughout the world, and a list of addresses of centers in charge of flight information regions (FIR) and/or upper flight information regions (UIR) is provided in the Location Indicators manual (Doc 7910) which can be purchased from:

The Manual of designators for aircraft operating agencies, aeronautical authorities and services (Doc. 8585) provides such a listing. The manual can be purchased from:

What is the procedure to obtain a three-letter code for an (aircraft) operating agency?

Only the Civil Aviation Authority of the State responsible for the certification (approval) of your operation can assist you. You submit your request to your authority, in accordance with the national regulatory requirements and the authority will in turn coordinate the allocation of your code with the Secretary General of ICAO in Montreal. ICAO will only entertain requests submitted by State authorities.

What is an international flight?

An international flight is a flight that contains one or more international flight stages;

and

an international flight stage is a flight stage with one or both terminals in the territory of a State, other than the State in which the airline is registered (technical stops are not considered in classifying flight stages).
